I am trying to get braces for my 11year old son who has 4 missing permanent teeth on the bottom. 2 from front and 2 from back of the mouth. He has some overbite problem too. I had 2 orthodontic consultation but they are very different from one another. One doctor wants to pull 2 teeth from upper teeth and adjust so he will have normal bite and 2 implants later. Second doctor wants to just adjust his teeth arrangement and correct the bite problem but he will need 4 implants later. Is pulling 2 healthy teeth now to have less implants better or leaving the teeth as it is now and get more implants later better?

That depends on many factors. Obviously the condition can be treated in several ways. Both may work out. I would not recommend either based on the information received in this note, however. I would want to know more before giving an opinion as to which is better. Looking at the patient and at the x-rays is critical to the diagnosis and treatment plan one would suggest.I have done orthodontics for 27 years and implants for 20 years, so there is a case to be made for each option. Anytime you are missing permanent teeth it is always better to replace those teeth with implants rather than to pull teeth and squeeze the existing teeth together. It is nearly impossible to create the correct bite by doing that. My suggestion would be to follow the second doctor''''s recommendation. In a case like this it is important to have an experienced dentist who has experienced specialists who work/communicate well together on cases like this. It sounds to me like we will want to work with an orthodontist who can get his teeth/jaws/bite properly aligned - then drop implants in at the appropriate time. At the same time - we need to make sure his gaps are filled with good-looking temporary teeth while he waits for the implant phase.
